FUNDING OPTIONS
We accept the following payment options: Self Pay, Medicaid Waivers, FSSP funding, HSA/FSA cards
Self Pay rates:
45 minute session = $90
60 minute session = $110
All self-pay patients are billed once a month at the end of the month. Online bill pay is available.
Medicaid waivers:
We accept the following Medicaid waivers:
Children’s Extensive Supports (CES)
Children’s Habilitation Residential Support (CHRP)
Supported Living Services (SLS)
Children with Complex Health Needs (CwCHN)
Enrollment for Medicaid waiver programs are completed through local Community Centered Boards (Weld Co, Foothills Gateway)
FSSP funding:
We accept the Family Support Services Program. All FSSP patients are billed at our self-pay rate. There are two billing options
1. Family submits an invoice to case management, and a check is written to Elevate.
2. The family pays the invoice and submits the receipt to case management.
